Adult and under 18 Kayak, Surfski, Dragon Boat, SUP sessions, events, competitions, warm up, on the above event - The above registered Participant can swim more than 50 meters in clothed!
The above Participant does not suffer from any Heart Conditions, Asthma and/or Epilepsy
The above Participant will wear a Life Vest will be worn at all Paddling Training, Team Building Events & Race Events. - I note and agree that each Paddler (participant) participates in the Paddling Training Session or Race Events does so solely at his or her own risk with understanding of these risks and will not hold siren events LLC (paddlershub watersport services LLC and ECRF) nor any of their respective employees, officers or agents liable for any personal injury or loss of property and in any way arising as a consequence of the participation in the Paddling Training Sessions, Team Building Events or Race Events.
- I agree to indemnify siren events LLC (paddlershub watersport services LLC and ECRF) or any of its members or agents against any claims, demands, actions, proceedings, judgments, liabilities, loss damages,
- costs and expenses (losses) arising from accident, injury (whether or not fatal), illness (including COVID-19) or damage to any person or property whatever in any way arising as a consequence of my minor son/daughter participating in any Paddling Training Sessions, Team Building Events or Race Events.
- I/ the legal guardian of the above Participant fully understand that paddling in /on a Dragon Boat, Kayak (Surfski), SUP is a dangerous sport and recreational activity which involves a number of significant risks of physical harm. Dragon Boating or kayaking (surfski), SUP has a number of obvious risks and a number of risks which are not so obvious, but which are nonetheless significant risks of physical harm. In engaging in Dragon Boating, kayaking(surfski)or SUP that the above participant will be exposed to the following risks: Death by drowning, death, permanent disability, paralysis, shoulder dislocation, rotator cuff injuries, broken limbs, blisters, back complaints (including disc prolapse), bruising, needle stick injuries, cuts, cuts from broken glass, viral and bacterial infections from water pollution, jellyfish sting, hypothermia,
- dehydration, sunstroke, skin cancer, exhaustion and lack of concentration. A number of the risks identified above are clearly obvious risks and are inherent in the sport of Dragon Boating or kayaking (surfski), SUP whereas a number of risks identified above arise from the environment in which the sport of Dragon Boating, kayaking (surfski) or SUP is conducted (for example, where and how we load the boats, the wharf etc). None of the risks should be ignored or taken lightly.
- I/ the legal guardian of the above Participant also fully understand paddling in a dragon boat or kayak (surfski) is a dangerous sport and have noted the warning with regard to the risk a participant is exposed to.
- I/ the legal guardian of the above Participant also confirm that we can swim more than 50 meters and do not suffer from any Heart Conditions, Asthma and/or Epilepsy or any diseases which can affect the safe participation of the training sessions.
- Photographic and or visual images & videography taken by siren events LLC (paddlershub watersport services LLC and ECRF) of my participation in the program may be used for general promotion of siren events LLC (paddlershub watersport services LLC and ECRF) activities.
